It looks like the main light is a beauty dish a little low looking at the highlight in the eye and edge lights on both sides and slightly behind the subject and a background light.......
along with the main light there might also be a fill light, but the highlight in the eye might have been photoshopped out.... just a guess.

To it looks like a clamshell lighting with added backlights. Top front light to high to be seen in the eyes or edited out.
