How to reset brush

I don't remember exactly what I had done but the diameter of my brush eg. healing brush is jagged around the whole circumference. Instead of painting with the whole brush, it seems like I'm painting with only the center of it. It should be smooth all the way around. Can someone please tell me how to reset it so that it is smooth all the way around and not jagged.
