Forms?

Do any of you, actively involved in retouching, have a form that I can beg, borrow or steal? i have the Katrin Easman book with it's form, but I doubt I will be doing the kind of detail she is. My area of concern wil be more site specific, as opposed to person specific. As a novice in this endeavor I am unsure what information I should have up front and am hoping one of you who has been doing this for awhile, might have some insights that could ease my transition into this. Many thanks.

I am referring to the one in her book, "PhotoShop Restoration and Retouching" page 233, entitled Digital Restoration and Retouch Request. I am planning on doing more site/background retouching, than I am on people retouching, or beauty enhancement. At least that is the plan now. This will probably be revised at some point, but my initial focus will be on scanning images, color balancing them, removing dust and scratches, sharpening the images, removing any color casts, repairing any damage to the images (rips, folds, etc.), removing unwanted items, adjusting the skyline/backgrounds, etc.
