Get a brush and remove "structure"

The only thing I can add to that is it looks like crap when you remove structure. This is mostly set up with lighting and makeup. You make sure the shadows are the right size and in the right place. You can always darken them a bit in post. You need large but very direct/parallel lighting.

You do smooth out unevenness in skin, but you don't really remove structure. I can tell you that your question misinterprets the source. If you try to remove structure, you end up with an image that looks terrible. No one wants that. You only think you want it, and if you asked that question, you're probably still very far from achieving it at this point.

At the risk of sounding pedantic - It's skin texture that has been removed, not structure. As I understand it, texture means surface quality, whereas structure is to do with 3D form.

The head in the picture almost looks like it has been pasted from another source. Not only is the tonal treatment very different, but also she would need to be performing a fairly extreme neck rotation to get that relationship to her right arm.

In my opinion, absence of texture it's necessarily wrong (I'm thinking for example of the long exposures of Paolo Roversi), nor is it necessarily wrong to reduce it in post provided there is a good reason for doing so, and it is done skilfully and with taste.
