I am thinking the same that a plugin may be used here for smooth retouching. As like someone has suggested above I am going to say the same that plugin like Portraiture or Portrait Magic Lite can do that.
This is definality portraiture plugin for photoshop. I use this software daily. Worth the money especially for basic retouch. However not for high end.

When you say "smooth," do you mean the skin, or do you mean literally "everything"? The skin has obviously been done, probably (as other have mentioned) with Imagenomic Portraiture. Other than that, I don't know what "smoothness" you might be referring to.

So many photographer alway retouch like it and I love it so but I don't how to retouch.
